Blocking key protein could halt age-related decline in immune system, study finds

ScienceDaily (Sep. 30, 2012) ? The older we get, the weaker our immune systems tend to become, leaving us vulnerable to infectious diseases and cancer and eroding our ability to benefit from vaccination. Now Stanford University School of Medicine scientists have found that blocking the action of a single protein whose levels in our immune cells creep steadily upward with age can restore those cells' response to a vaccine.

This discovery holds important long-term therapeutic ramifications, said Jorg Goronzy, MD, PhD, professor of rheumatology and immunology and the senior author of a study published online Sept. 30 in Nature Medicine. It might someday be possible, he said, to pharmacologically counter aging's effects on our immune systems.

In the study, the Stanford team fingered a protein called DUSP6 that interferes with the capacity of an important class of immune cells to respond to the presence of a foreign substance, such as those appearing on the surface of an invading pathogen or in a vaccine designed to stifle that invasion.

The researchers also identified a potential lead compound that, by inhibiting DUSP6's action, restores those cells' responsiveness to a more youthful state.

A person's immune response declines slowly but surely starting at around age 40, said Goronzy. "While 90 percent of young adults respond to most vaccines, after age 60 that response rate is down to around 40-45 percent. With some vaccines, it's as low as 20 percent." Vaccine failure among seniors poses a serious health problem: Some 90 percent of influenza deaths are among people over age 65.

A vaccine is, in essence, a "mug shot" of one or more of a pathogen's most prominent features, akin to a photo of a giant wart on a suspect's nose. This chemical snapshot -- or antigen, in scientific parlance -- is nailed into customized "frames" and displayed on the surface of "desk cop" cells specializing in signaling T cells, the "beat cops" of the immune system.

One important type of beat cop, the so-called T-helper cell, stimulates other immune cells called B cells that play a key role in our response to infection-preventing vaccines. On exposure to an antigen from a vaccine or the pathogen itself, along with appropriate interactions with T-helper cells, B cells proliferate, mature and get down to brass tacks: producing and secreting antibodies. These molecules are designed to snare and immobilize the pathogen, flagging it for incarceration and, quite likely, a death sentence meted out by still other very tough immune cells.

For poor responders, there are a few ways of increasing a vaccine's potency. One is to simply boost the dose. A second is the use of adjuvants: chemicals, or combinations of them that, like a cup of strong coffee, wake up the desk cops whose job is to display the description of the pathogen's distinguishing feature (the antigen) to the beat cops. A recently published large, multicenter study, one of whose co-authors was Cornelia Dekker, MD, professor of pediatrics (infectious disease) at Stanford, showed that adding an experimental adjuvant to the standard seasonal flu vaccine substantially improved seniors' response to the vaccine.

But just waking up the desk cops won't cut it if the beat cops are too sluggish. Older people's T-helper cells suffer from a diminished capacity to activate, proliferate and secrete crucial signaling chemicals in response to infections or vaccines. This limits even an adjuvant-containing vaccine's ability to get the job done.

"Some age-associated defect or defects raise the threshold of responsiveness to the presented antigen, so a vaccine dose that triggers T-cell activation in a younger person doesn't in an older person. Adjuvants can't compensate for these defects," Goronzy said.

So he and his team sought to identify the defects that cause this age-related sluggishness in T-helper cells, and to see how to counter them.

Circulating T-helper cells fall into two broad categories. "Na?ve" T-helper cells have never encountered an antigen before (as in the case of, say, a rare or emerging pathogen or a new vaccine), but are capable of wheeling into action once they do. It takes a week or two to reach full tilt.

"Memory" T-helper cells have previously been exposed to an antigen. These cells are long-lived and narrowly fixed on that particular antigen. They can rapidly transition to an activated state should the same antigen ever cross their path again. That's why prior exposure -- through infection or a vaccine -- renders us more resistant.

In a Proceedings of the National Academy of Sciences study published earlier this year, Goronzy's team showed that faulty regulation in memory T-helper cells, due to aging-related increased levels of a protein called DUSP4, inhibits the activation of those cells, with their consequent failure to ignite a good B-cell (antibody-producing) response.

This time around, the investigators uncovered a similar effect with a related protein, DUSP6, on na?ve T-helper cells. In test tubes, they compared blood cells drawn from people ages 20-35 versus 70-85 in response to stimulation. In na?ve T-helper cells (but not in memory cells), there were age-associated differences in a specific chain of biochemical events involved in the cells' activation, proliferation and differentiation. Laboratory tests showed that the culprit behind the cells' fecklessness in older people was DUSP6, an enzyme that works by hacking phosphate groups off of other enzymes, thus dialing down their activity. Those "downstream" enzymes are crucial to na?ve T-helper cell activation. DUSP6 levels were much higher in older people's na?ve T-helper cells.

Further experimentation revealed that DUSP6's increase in aging na?ve T-helper cells was caused by an age-associated easing up on a brake pedal called miR-181a, one among hundreds of small molecules made of RNA (called microRNA) that regulate proteins' production. All microRNA molecules work by affixing themselves to larger protein-template RNA molecules, also made of RNA, gumming things up and stalling the procedure. Each distinct variant of microRNA molecule can bind to several different varieties of protein-prescribing RNA molecule, thereby swiftly redirecting a cell's overall behavior.

Goronzy and his colleagues saw that miR-181a directly interferes with the production of DUSP6 and noted that the amount of miRNA-181 present in na?ve T-cells declines steadily, bottoming out around age 65-70, causing levels of DUSP6 in these cells to increase with age. Artificially boosting miRNA-181a levels in na?ve human T cells caused DUSP6 levels to plummet, commensurately increasing those cells' readiness to activate on exposure to a given dose of influenza vaccine. In contrast, artificially increasing the levels of DUSP6 blocked the beneficial effects of heightened miR-181a levels.

A study conducted by University of Pittsburgh researchers and published in Nature Chemical Biology in 2009 had shown in zebrafish that a particular compound with an extremely long chemical name (abbreviated in Goronzy's study as "BCI") appeared to block DUSP6's action in certain heart cells, leading to cardiac hypertrophy. (In this case, DUSP6's proliferation-preventing function was beneficial.)

So Goronzy and his colleagues incubated blood cells from 10 60- to 85-year-old individuals with activation-stimulating molecules in the presence of increasing concentrations of BCI. The greater the BCI dose, the more evidence of na?ve T-helper cell activation they saw.

"We are still far from application in the clinic," cautioned Goronzy. "We need to keep tweaking the compound and testing it in mice to make absolutely sure it's safe enough to try in humans. But improving vaccine responses to overcome age-related immune defects represents a unique opportunity to attain healthy aging."

First authorship of the study, funded by the National Institutes of Health, was shared by Guangjin Li, PhD, Mingcan Yu, PhD, and Won-Won Lee, PhD, all postdoctoral scholars in Goronzy's lab. Other Stanford co-authors were Eswar Krishnan, MD, assistant professor of medicine; and Cornelia Weyand, MD, PhD, professor of medicine.

Drew Brees and Aaron Rodgers were both outstanding on Sunday at Lambeau Field, but in a back-and-forth game with plenty of twists and turns, it was Rodgers? Packers who ended up on top of Brees? Saints, 28-27.

Brees completed 35 of 54 passes for 446 yards, with three touchdowns and no interceptions, but that wasn?t enough. Although he maneuvered them into range for an apparent go-ahead field goal late in the fourth quarter, the Saints? successful kick was called back on a holding penalty, and when they were backed up the Saints missed.

Rodgers completed 31 of 41 passes for 319 yards, with four touchdowns and one interception, and a short completion on a third down just after the two-minute warning iced the game and gave the Packers the win.

For the second consecutive week, the Packers suffered at the hands of bad officiating. Last week it was the infamous Hail Mary in Seattle that wrongly gave the Seahawks a touchdown and likely led to the end of the NFL?s lockout of its referees. This week there are no replacements to blame, just longtime ref Jeff Triplette and his crew, who botched several calls, including a fumble by Saints kickoff returner Darren Sproles that they wrongly ruled down by contact, and a push-off in the end zone by Saints receiver Marques Colston on a touchdown catch that should have been called back for offensive pass interference.

But this time the bad officiating didn?t cost the Packers the game, and they improved to 2-2. The Saints fought hard, but in the end they lost once again, falling to 0-4.

Building a Winning Business Analyst Team Through TrainingIn the spirit of the football season, I wanted to begin this blog post with a quote from famed Green Bay Packers football coach Vince Lombardi: ?Leaders are made, they are not born. They are made by hard effort, which is the price which all of us must pay to achieve any goal that is worthwhile.?

I think the same could be said about good business analysts. Business analysts are made (through training and effort), not born. While rising demand for BAs has led many universities to begin offering majors in Business Analysis and related fields, the same combination of factors is also causing many people who have little or no formal BA training to be thrust into the role.

A person may be considered a competent BA for many reasons. One of the best BAs that I have worked with used her familiarity with the organization to help her drive requirements gathering toward successful completion.?She knew how to negotiate between different stakeholder groups and knew just who to ask to get her questions answered.

There is no doubt that having in-depth knowledge of the organization is an asset to a BA. But that is also the problem with this method. By definition, this kind of familiarity takes a long time to develop and cannot be easily imparted on to other BAs. If you have it, great, but not everyone will, and it takes a long time to develop.

Other competent BAs possess great communication skills. They know the right questions to ask and can think quickly on their feet. Still others can use their technical knowledge to identify issues that may arise during development and catch them early. Working with different BAs has impressed upon me the myriad of styles and approaches that can be used to get the job done, and well.

BUT?woe to the organization that relies solely on the natural talents of their BAs. To continue the football analogy, a guy may be 6? 4? and 235 lbs of solid muscle, but if no one taught him how to tackle, chances are he won?t be a very good linebacker. And even if he has learned how to tackle, that skill won?t be very useful unless he understands when to tackle the guy, how to get off of a block, what his responsibilities on the field are, and how he fits into the schemes that his team is running!

By the same token, good BAs understand that there are many skills out there that are necessary to be successful, and they understand what their main responsibilities are in relation to those of the other members of the team. Unlike the football player who knows only how to play one position in one formation, a truly valuable BA can draw upon an array of business analysis techniques and skills. These skills will help them meet the challenges of any situation, function successfully in any division of the organization, and work effectively as a member of the team.

Bass are doing well in the river. Catfish are starting to pick back up.

Mike at Pat's Sporting Goods (804 748-4165) reports crappie fishing is turning on in the river and ponds on small minnows and 2-inch jigs. Stripers are back at the Benjamin Harrison Bridge and are hitting rattle traps, red fins and buck tails.

Notable catch: Danny Poh, 36-pound blue cat.

Capt. Art Conway of Conway's River Rat Guide Service (804-746-2475) out of Ed Allen's Boats and Bait reported Chickahominy Lake midday main lake water temperatures in the low to mid-70s on Wednesday. Temperatures were slightly lower up the lake. The lake level was a few inches above the top of the dam. The water was brown but relatively clear in the lower lake and was noticeably clearer in channels up creeks.

Fishing with Capt. Conway, Capt. Bill Buck and Hollis Pruitt caught 53 bluegill, 15 crappie, one shiner and a bass.

Jill O'Brien-Jones at Eagles Landing (804 966-9094) reports the water level is slightly below normal with temperatures in the upper 70s.

OUTER BANKS

Dr. Julie Ball reports sight casters are finding decent cobia action using eels and jigs. Nice fish are hanging around lower bay and inshore buoys.

Flounder action is steady in the lower bay, especially from the 3rd island area.

Tautog are beginning to bite inside the Chesapeake Bay and along the Chesapeake Bay Bridge-Tunnel tubes and structure.

Big red drum bite continues to spread throughout the lower bay and coastal waters. Reds of more than 54 inches are providing action along the breakers of the Eastern Shore barrier islands and inlets.

Speckled trout are making a good showing within the Eastern Shore Bayside creeks, with Hungar's Creek and Nassawadax Creek giving up consistent catches of bigger fish.

The action in Lynnhaven Inlet is improving with scattered fish to 22 inches, while Rudee Inlet has some decent fish taking grubs and Gulp mullets.

Blueline tilefish, golden tilefish, and a variety of grouper and other deep bottom dwellers are available along the edges of the Norfolk Canyon. Chopper bluefish will become active on offshore wrecks soon, with the Triangle wrecks a favorite.

Dr. Ken Neill III of the Peninsula Salt Water Sport Fisherman's Association reports amberjack are available at the southern towers and over some of the ocean wrecks.

The coastal wrecks are covered up with sea bass and triggerfish.

Big sheepshead continue to be available at the CBB-T.

There's a little mandolin, and there's a little fiddle hidden in there, but mostly it's the guitar magic of Ben Mink, our producer, who knows how to tape things. I think it's the most looped, constructed-sounding thing on the whole album - basically a lot of cool trickery.

You have 12-year-old twin boys that you've said are "going on 14." Do you take them on the road?

They come out in the summertime and ride the tour buses with us. While Ann and I go out on stage to do a sound check, or get our hair and makeup ready backstage, the boys will go see a movie or hit a museum, then come watch the show.

Do you introduce your kids to certain music?

I play a lot of classical music around the house. When the?Beatles Rock Band?video game came along, they learned all the songs. They'd wake up the next morning singing "Nowhere Man," and I was like, "Thank you, modern technology!" Now they're discovering their own music.

I decided not to push music on them. They're both naturals at music, but I want it to be an organic outgrowth. In fact when people ask me what advice I'd give aspiring musicians, I usually say, "Turn back before it's too late!"

Has your approach to melody and harmony changed over the years?

What I learned scoring films [Jerry Maguire, Almost Famous?and?Vanilla Sky,?all written and directed by Wilson's ex-husband, Cameron Crowe] was invaluable. As a songwriter, simplicity ? what not to do, what not to play ? can be the hardest thing to achieve.

I think you have to take all the colors in the palette of your life and make one great painting while you still can. Try not to keep finishing it ? just let it be finished.

Do you discipline yourself to write every day? Or just wait for the muse to strike?

Our lives are so chaotic that it's situational for both Ann and me. We're working moms ? our business trips just happen to be rock tours ? so there's no way to approach songwriting methodically. It's really just a matter of keeping your ears (and your soul, your heart, your mind and your eyes) open for the muse. If the muse floats into your space, you stop whatever you're doing and write it down.

You and Ann have an upcoming book, "Kicking and Dreaming."

It's very exciting.Our coauthor [Charles R. Cross] interviewed us individually and together for more than a year. We also sat down with our sister Lynn and talked a lot as three sisters together. We even went into the studio and recorded the story as an audiobook. Both looking back and projecting into the future, it's been a big year for us!

Artists that broke in during the 1970s have watched their audiences gray over the years, but Heart seems to attract a younger crowd, too. Have you noticed that?

Yeah ? and it's cool that it's happening to us. Ever since Carrie Underwood sang "Alone" on?American Idol?and [the video game]?Guitar Hero?featured "Barracuda," younger and younger kids have been coming to our shows. When a song takes on a life of its own like that, it's the ultimate compliment for a songwriter: It lets you connect the dots between our generation and the next ? and maybe even the next!

But all of those phones started out with a much higher price, unlike AT&T's latest budget smartphone, the LG Escape, which is available for just $49.99 with a two-year contract.? Thankfully, this affordable price tag doesn't come at the cost of performance or features. The LG Escape has a powerful processor, blazing fast LTE data speeds, a bright, attractive display, and a thin design. Along with our Editors' Choice, the Pantech Flex, it's the model for what a new budget smartphone should look like.

Design, Connectivity, and Call Quality
Attractive in a slim, minimalist way, the Escape measures 4.96 by 2.54 by 0.37 inches (HWD) and weighs just 4.5 ounces. It's made of shiny black plastic with a light pattern printed on the back and a smoked chrome ring around the battery cover and display. The only false note is the textured, matte plastic band around the middle that looks a bit tacky. Otherwise, this looks and feels like a quality device.

The rest of the phone's design is understated as well. There's a volume rocker on the left edge, a Power button on the right, a 3.5mm headphone jack on the top, and a microUSB port on the bottom. That's it. There's about half an inch of bezel on either side of the 4.3-inch, 960-by-540-pixel IPS LCD, which looks beautiful. It gets very bright, and its 256 pixel-per-inch density makes for sharp text and images. The phone is light and comfortable to hold, and typing in both portrait and landscape mode is fast and fluid.

The Escape runs on AT&T's EDGE, HSPA+ 21, and LTE networks, and features 802.11b/g/n Wi-Fi. Reception is strong, and like most AT&T LTE devices we've tested so far, the Escape pulled in some incredibly fast data speeds, with downloads averaging 18Mbps, and uploads just over 12Mbps. If you take a look at our Fastest Mobile Networks?survey, you'll see that AT&T's LTE can exceed Verizon's LTE speeds, though Verizon's LTE is more consistent and covers more of the nation.

Call quality is mostly good. In my tests, voices sounded a little crackly in the earpiece, with a slight echo in the background, but were otherwise loud and easy to hear. Calls made with the phone are extremely clear, and average noise cancellation helps to block out some, but not all, background noise. Calls sounded great through a Jawbone Era?Bluetooth headset and the stock Android voice dialer worked fine. The speakerphone sounds okay, and is just about loud enough to hear outside. Battery life was average at 6 hours and 27 minutes of talk time.

Processor, Android, and Apps
The dual-core 1.2GHz Qualcomm Snapdragon S4 MSM8960L processor is no slouch. It isn't quite as scorching as the dual-core 1.5GHz S4 in the Samsung Galaxy S III, but it's fast enough to keep all the apps and games you want to play running along at a nice clip. You also don't have to worry about it becoming totally obsolete in the next few months.

The LG Escape is running Android 4.0.4 "Ice Cream Sandwich," along with LG's Optimus UI overlay. It's a fairly light skin, but it makes the phone feel a little more accessible and user-friendly than stock Android. Unfortunately, there's no word on an update to Android 4.1 "Jelly Bean," which seems to be the case for most current Android phones.

5 Different Kinds Of Automotive Training

When you hear the words automotive training, what springs to mind? Some colleges that offer automotive training also offer classes for aspiring transportation operations specialists. Trucking is a vibrant, stable industry in North America, with a lot of job opportunties. Canada in particular is in need of an influx of young people taking up the profession.

BlackRock raises S&P 500 target, but says rally over

NEW YORK (Reuters) - BlackRock Inc , one of the world's largest asset managers, has raised its expectations for the U.S. stock market in 2012 but believes this year's equity rally has run its course, according to targets provided by the firm on Tuesday.

The Standard & Poor's 500 index <.spx> will finish 2012 at 1,450 and rise to 1,525 by mid-2013, according to Chris Leavy, BlackRock's chief investment officer, fundamental equities of the Americas.

The new targets are up from the firm's earlier forecasts and reflect a 7 percent rally in the benchmark index since early August. Earlier, the firm projected the S&P 500 would finish the year at 1,350 and rise to 1,400 by mid-2013.

BlackRock provided the targets to Reuters, which is releasing its broad global equity poll on Wednesday.

The firm had assets under management of about $3.56 trillion on June 30, including $297 billion in equities under management.

Late Tuesday afternoon, the S&P 500 was down 0.5 percent at around 1,450, where BlackRock says it will finish the year, but 4.8 percent below its target for mid-2013. The index reversed earlier gains around midday after BlackRock updated its targets.

The new targets are the first from Leavy, who took over management of the firm's large-cap series funds in June from Bob Doll, the firm's chief equity strategist. Doll, a 30-year industry veteran, is in the process of retiring from the firm. Leavy has already taken over responsibility for the firm's large-cap series funds.

Leavy was not immediately available for comment.

Modern care and a bit of shopping is just what the doctor ordered.

An increasing number of wealthy Chinese are flying overseas, but not to shop for luxury brands such as Louis Vuitton, Gucci and Prada. These people are looking for something not found on department store shelves. They are seeking beautiful faces, stronger hearts, clearer eyes or just a better understanding of their health.

Liu Yuan, a 31-year-old sales manager at an advertising company in Beijing, had laser eye surgery earlier this year. She was impressed with the care and the skill of surgeons at the private clinic she attended in Singapore.

"The long waiting time at public hospitals in China is so embarrassing and you can't choose the surgeon you want. And laser eye surgery in Singapore has a good reputation," Liu said.
The return flight cost her 5,000 yuan ($793) and she also had the opportunity to shop in Singapore. "It's a double-gain trip," Liu said.

She is one of the thousands of Chinese residents who choose to venture abroad for medical treatment. With the rising affluence and mobility of the country's emerging middle class, there's been a significant increase in the numbers of Chinese traveling overseas as medical tourists in the past decade.

Around 60,000 Chinese people travel abroad annually for healthcare services, especially for anti-aging therapy, cancer screening, to give birth and for treatment of chronic diseases, according to Yang Jian, CEO of the Shanghai Medical Tourism Products and Promotion Platform.

In January 2008, Shao Hui learned that his sister had been diagnosed with a 5 millimeter diameter tumor in her lung.
The diagnosis came from a hospital in Japan, which has some of the best cancer-screening technology in the world. Just a few months later, the same hospital told the 36-year-old businessman that he had cancerous growth in his stomach.

The results were beyond Shao's expectations. "We have annual health checks at one of the best hospitals in Beijing and they didn't reveal any problems," he said.

His sister decided to act on the suggestion of the Japanese physicians and have the tumor excised, but not to undergo any other treatment such as radiation or chemotherapy. The Japanese doctors also guaranteed that the cancer would not reappear.

"I talked with some doctors in Beijing and they were astonished. It's highly unlikely that such a small tumor could be detected in a hospital in China, and the doctors would never promise that there was no chance of a recurrence," said Shao.

Shao himself underwent a gastroscopy, an examination of the stomach and alimentary canal, in Japan. The treatment left a good impression on him. The procedure, usually performed through the mouth in China, is performed nasally in Japan, significantly reducing patient discomfort.

Shao and his sister were screened as part of a trial for his company, L'Avion, a travel agency that specializes in private medical tourism. Two years after the company was founded, Shao's family traveled to Japan for medical checkups in 2008. He later included the tour in the company's routes because of the treatment he had received.

Clients, most of whom are entrepreneurs aged from 40 to 50, pay thousands of yuan for overseas consultations and hundreds of thousands for surgery. At the high end, the service can include interpreters, tour guides, a first-class air ticket, a private driver and specially prepared Chinese or local cuisine. The company's most popular routes are anti-aging treatment in Switzerland and cancer screening in Japan.

Last year, the number of individuals in the Asia-Pacific region with more than $1 million at their disposal rose 1.6 percent to 3.37 million, according to the seventh Asia-Pacific Wealth report compiled by the consultancy Capgemini and RBC Wealth Management. Moreover, 17 percent of Asia's millionaires are located in China.

"Some, but still very few, of those wealthy people are aware of the importance of disease prevention. We are trying to promote the idea of enhancing the quality of life, rather than simply treating disease," said Shao.

Sheep serum therapy

Xiao Bo is familiar with medical treatment overseas. The 45-year-old from Jilin province in Northeast China works in international trade and spends most of the time in Beijing. A decade ago she started to pay regular healthcare trips to foreign countries, including the United States, Japan, Germany and Switzerland, seeking better technology and services than those provided in China.

Earlier this year, she paid 380,000 yuan for a trip to the Clinique Biotonus Bon Port in Geneva, Switzerland, where Nelson Mandela, the former president of South Africa, was treated in 1992. The clinic mainly provides treatment for intense fatigue, burnout and problems linked with aging.

The purpose of Xiao's trip was sheep placenta treatment, which she first had two years ago. The treatment is typically used to produce clear and healthy skin, free of wrinkles and blemishes. "Just compare the photos before and after the therapy, you can see a more youthful appearance and I feel I have more energy. The injection of serum has also boosted my immune system and I've been almost illness-free for the past two years," she said.

Chinese patients need to be better informed about the impact of lifestyle. They need to learn the value of good nutrition and exercise and to be aware of the importance of preventative medicine, said Mara Bianchi, assistant director at the clinic.

She said that the clinic has received more Chinese patients since 2006, when it started its cooperation with L'Avion. The influx of Chinese patients has brought changes to the clinic. Most of the physicians and nurses can speak at least two or three sentences in Mandarin and the menu now includes Chinese favorites such as porridge and pickled cabbage.

Growing demand

An aging population and rising incomes have increased the demand for medical and healthcare products and services throughout China. According to the Ministry of Health, mainlanders aged 60 or older accounted for 13.3 percent of the population in 2010, an increase on the 10.3 percent recorded 10 years ago.

"It is understandable that people choose to get medical treatment overseas. Generally speaking, Western countries are better off than us in terms of economics, medical technologies, and care services," said Liu Zhongjun, director of the Orthopedics Department of Peking University No 3 Hospital in Beijing.

For the wealthy, receiving treatment overseas may provide mental relief, if the medical care, services and hospital environment are taken into consideration. However in Liu's opinion, the price of treatment is too high - as much as a dozen times more expensive than in China - and far beyond the reach of the average person, said Liu.

"If some people think that overseas doctors have better skills than their Chinese counterparts, they are wrong," he said. "Doctors in the larger hospitals not only have up-to-date skills, facilities and equipment, but also a broader view of individual cases than their counterparts overseas."

"I will not go abroad for treatment if I become ill," he added.

New wealth in Asia

Medical tourism has become a boom industry in the past decade, not only in China, but the whole of Asia, mainly because of the emergence of new wealth in the region.

Industry experts predict that medical tourism in Asia will grow at a rate of 15 to 20 percent a year, according to reports from Reuters. Meanwhile, the Medscape News website predicts that the practice could generate $4.4 billion in the region this year.

That raises the question of whether competition from foreign providers might help to lower the cost of private medical treatment in China, but Deng Kaishu, vice-president of the Chinese Medical Doctor Association, said that's unlikely to happen in the foreseeable future because the number of patients is too small to make a difference.

"But the value of treatment for foreigners in China will inject new vitality into the domestic medical system," he said. "Traditional Chinese Medicine has great influence on patients overseas and we should better promote these services to attract them," he said.

Some medical service companies in China are making efforts to keep patients at home. Ciming Health Checkup Group, a leading private healthcare agency in China, set up the Oasis International Wellness Club in Beijing in mid-2011.

Membership services include free, extensive health checkups, tailored health management guidance, private doctors and overseas medical tourism services. Its clients, currently numbering around 50, are all Chinese and pay at least 120,000 yuan for a year's membership and around 3 million yuan for life membership.

Xu Qiong, a publicity officer for the group, says only 10 percent of clients aged between 35 and 55 have conditions that require surgery. "Most have non-life-threatening conditions and traditional Chinese medical treatment will always be the first choice. Thus, high-end medical services in China still have great potential," he said.

BEFORE he died on Sept. 8, Adnan Farhan Abdul Latif had spent close to 4,000 days and nights in the American prison at Guant?namo Bay, Cuba. He was found unconscious, alone in his cell, thousands of miles from home and family in Yemen.

Eleven years ago, he found himself in Afghanistan at the wrong place and the wrong time. It was an unusual set of events that took him there. Years earlier Mr. Latif had been badly injured in a car accident in Yemen. His skull was fractured; his hearing never quite recovered. He traveled to Jordan, seeking medical treatment at a hospital in Amman; then, following the promise of free medical care from a man he met there, journeyed to Pakistan, and eventually to Afghanistan.

Like so many men still imprisoned at Guant?namo, Mr. Latif was fleeing American bombing ? not fighting ? when he was apprehended by the Pakistani police near the Afghan border and turned over to the United States military. It was at a time when the United States was paying substantial bounties for prisoners. Mr. Latif, a stranger in a strange land, fit the bill. He was never charged with a crime.

The United States government claims the legal authority to hold men like Mr. Latif until the ?war on terror? ends, which is to say, forever. Setting aside this troubling legal proposition, his death and the despair he endured in the years preceding it remind us of the toll Guant?namo takes on human beings.

Adnan Latif is the human face of indefinite detention.

In the landmark 2008 case Boumediene v. Bush, the Supreme Court ruled that Guant?namo detainees were entitled to ?meaningful judicial review? of the legality of their detentions, via the writ of habeas corpus ? a constitutional check obligating the government to demonstrate a sufficient factual and legal basis for imprisoning someone. The Boumediene decision, in principle, ought to have given hope to Mr. Latif and men like him.

And it was under such principle that two years later, a United States District Court judge hearing Mr. Latif?s habeas corpus petition ordered him released, ruling that the accusations against him were ?unconvincing? and that his detention was ?not lawful.? By that time, Mr. Latif had been cleared for release from Guant?namo on three separate occasions, including in 2009 by the Obama administration?s multiagency Guant?namo Review Task Force.

Nevertheless, the Department of Justice appealed the district court?s decision to the United States Court of Appeals for the District of Columbia Circuit ? which has ruled in the government?s favor in nearly every habeas corpus appeal it has heard. The appellate court reversed the trial judge?s release order, effectively ruling that evidence against detainees must be presumed accurate and authentic if the government claims it is.

A strong dissenting opinion criticized the appellate court majority for not just ?moving the goal posts,? but also calling ?the game in the government?s favor.?

But Mr. Latif didn?t see it as a game. He was dying inside. Like other men, he had been on a hunger strike to protest his detention. After losing the appeal of his case, he told his lawyer, ?I am a prisoner of death.?

Three months ago, the Supreme Court declined to hear the appeals of Mr. Latif and six other detainees, who pleaded for the court to restore its promise of meaningful review of their cases.

But what is unsaid in all of the court rulings is that Mr. Latif was imprisoned not by evidence of wrongdoing, but by accident of birth. In Guant?namo?s contorted system of justice, the decision to detain him indefinitely turned on his citizenship, not on his conduct.

With Mr. Latif?s death, there are now 56 Yemenis who have been cleared for release by the Guant?namo Review Task Force since 2009 but who remain in prison. President Obama, citing general security concerns, has imposed a moratorium on any and all transfers to Yemen, regardless of age, innocence or infirmity.

It is fair, and regrettable, to assume that some of these detainees will die there as well.

Mr. Latif, after all, was the ninth man to die at Guant?namo. More men have died in the prison camp than have been convicted by a civilian court (one) or by the military commissions system in Guant?namo (six). In 2006, Salah al-Salami, a Yemeni, and Yasser al-Zahrani and Mani al-Utaybi, both Saudis, were the first men to die at Guant?namo. Their deaths were called suicides, even though soldiers stationed at the base at the time have raised serious questions about the plausibility of the Defense Department?s account. (Full disclosure: the Center for Constitutional Rights represents the families of two of the men who died.)

According to the government, three more detainees committed suicide and two others died of natural causes. There has been no independent investigation into any of the deaths, however; there has been no accountability for a range of constitutional and human rights violations at Guant?namo.

The government has not yet identified the cause of Mr. Latif?s death, but it is Guant?namo that killed him. Whether because of despair, suicide or natural causes, death has become an inevitable consequence of our politically driven failure to close the prison ? a natural byproduct of the torment and uncertainty indefinite detention inflicts on human beings.

The case of Adnan Latif should compel us to confront honestly the human toll of the Guant?namo prison ? now approaching its 12th year in operation. We can start this reckoning by releasing the 86 other men at Guant?namo who the United States government has concluded no longer deserve to be jailed there.

CNN questioned over use of slain Libya ambassador's journal

A personal journal belonging to U.S. Ambassador to Libya Christopher Stevens was obtained by CNN, and The Wall Street Journal is reporting that it "broadcast reports based on its contents against the wishes of the Stevens family, according to relatives and State Department officials who were asked to intervene by the family."

The cable news network didn't publicly acknowledge having the journal until Friday night.

According to an un-bylined story posted Saturday on CNN's website "four days after he was killed, CNN found a journal belonging to late U.S. Ambassador to Libya Chris Stevens. The journal was found on the floor of the largely unsecured consulate compound where he was fatally wounded."

Stevens and three other Americans were killed in an attack by militants in the American consulate in Benghazi on Sept. 11.

CNN claims on its website that it "notified Stevens' family about the journal within hours after it was discovered and at the family's request provided it to them via a third party."

The Journal reports that the slain ambassador's family contacted the State Department, which put together a conference call between the Stevens family and CNN. During the call, the family "personally appealed to the news organization to return the journal and to not publish or broadcast any of its contents, according to a Stevens family member and State Department officials."

The family also told CNN that they wanted to look at the seven-page handwritten journal before the news organization used it, "saying the ambassador's personal writings belonged to them." The Journal reports that CNN agreed to not use the contents of the diary until the family had had a chance to look at it themselves.

The Journal reports that State Department officials arranged to have CNN give the journal to an Italian diplomat in Benghazi, using a third party because U.S. diplomats had evacuated the city after the Sept. 11 attack.

On Wednesday, CNN talk show host Anderson Cooper told Sen. John McCain on "Anderson Cooper 360," that "a source familiar with Ambassador Stevens's thinking told us that in the months before his death he talked about being worried about the never-ending security threats that he was facing in Benghazi and specifically about the rise in Islamic extremism and growing al Qaeda presence." The source, Cooper said, "also mentioned (Stevens) being on an al Qaeda hit list."

The Journal reports that "a spokesperson for CNN said the network didn't report directly from the journal, but corroborated the information through other sources."

Cooper acknowledged on his show Friday night that CNN had obtained Stevens's journal and it had played a role in the network's reporting.

"Some of that information was found in a personal journal of Ambassador Stevens in his handwriting," Cooper said Friday. "We came upon the journal through our reporting and notified the family. At their request, we returned that journal to them. We reported what we found newsworthy in the ambassador's writings."

CNN was contacted Friday afternoon by the Huffington Post, but the network chose not to comment. CNN referred HuffPo to Cooper's comments after they aired.

The Journal says the State Department disputes CNN's account. It goes on to report that "Philippe Reines, a senior adviser to Secretary of State Hillary Clinton, said it took repeated prodding to get CNN to agree to return the journal."

Why you need auto insurance!

Besides the purchase price of a car, one must also calculate the potential maintenance cost, and auto insurance cost. We often focus on the first two costs, but seldom do we realize the auto insurance cost differential until we?ve purchased the car!

I was so happy to negotiate $2,000 off the initial asking price of a $14,000, black on black, BBS wheels equipped 1997 M3 in 2003 that I didn?t realize my auto insurance would jump from just $500 a year to $2,000 a year!

Had I realized my auto insurance would jump so much, I probably would have negotiated even harder, or reconsidered buying a sports car altogether. Always get an auto insurance quote before you purchase a car please!

Having auto insurance is a law you don?t want to break. Accidents happen all the time whether they are your fault or not. If you end up wrecking someone else?s car and injuring a passenger, not having any car insurance could literally wipe you out financially and put you in poverty for the rest of your life!

AUTO INSURANCE BASICS

There are three things you want to protect with auto insurance in case of an accident: 1) You and your passengers, 2) Your assets, and 3) Your vehicle. Most people would choose protecting you and your passengers as the most important purpose of auto insurance. I would too. However, it all depends on how wealthy you are.

1) Insurance To Protect Your Financial Resources

Let?s say you are multi-millionaire who employs 10,000 people who depend on your existence to put food on the table. If you go down, so does the company. Protecting your financial assets might be more important than protecting you and your passengers sad to say! If you nod off and accidentally kill several pedestrians on the sidewalk, you can be sure you will be in a legal battle for multi-millions of dollars. You will be all over the news and you won?t be able to focus on running your company.

If you did the same thing and were poor, the victims or family of the deceased can?t squeeze blood from stone. As a result, you care less about protecting your own financial resources since you have little, and care more about protecting the passengers in your car. Of course, if you are the multi-millionaire, then you can afford to care for everything and get the most protection possible.

Bodily Injury Liability:?If you hurt someone in an auto accident, the insurance company will pay a certain amount.? In my case, I elected for $300,000/$500,000 per person/per accident.

Property Damage Liability:?If you damage someone else?s property in an auto accident, the insurance co will pay a certain amount. My insurance company pays $100,000 per accident to property.

2) Insurance To Protect You And Your Passengers

You can be the most careful driver in the world, but that is only half of the equation. If someone else is careless, they can easily crash into you and cause damage. Even though auto insurance is mandatory for all drivers, many drivers still do not have auto insurance! As a result, your insurance needs to protect you from others.

Uninsured Motorists Bodily Injury:?If an uninsured or underinsured driver injures you or your passengers. I elected $300,000/$500,000 per person/per accident.

Medical Payments:?Covers you or your passengers. If someone runs into you, or you run into someone else and have an injured passenger, you want to be able to provide financial coverage for any medical expenses they may incur. Hopefully, your passengers will not sue you if you don?t have such coverage, but it?s better to be safe than sorry. I have $100,000 per person.

Extended Benefits:?Wage earner disability benefits, essential services disability benefits, death benefits. If an auto accident causes so much physical or mental pain that you cannot return to work, your extended benefits beyond your company?s short term disability benefits will kick in if you do not have long term disability insurance. I pay $5.10 for minor distribution of all three. I have my life insurance and disability insurance from work to cover 100%.

3) Insurance To Protect Your Vehicle

One of the most wonderful things about driving a 12 year old vehicle is that it doesn?t cost much to replace. Moose, my SUV is probably worth no more than $3,500 anymore. If I was to get in a car accident, the cost to fix whatever part of Moose will probably cost at least $1,000. At this point, it?s better to just donate him to charity and consider buying another vehicle. As a result, I do not have comprehensive insurance.

Comprehensive: Covers damage caused by something other than another vehicle such as hail, theft, and collision with Bobo the bear. If you have a shiny new $50,000 BMW, you should get comprehensive insurance because you?ll want to fix it in an accident and you won?t want to pay thousands of dollars to do so. The more expensive your car and the more dependent on your car, the more you should consider getting comprehensive insurance.

Collision:?Covers damage to your vehicle caused by impact with an object other than an animal. Gray area here, given this is what comprehensive is for. I declined coverage.

Uninsured Motorists Property Damage:?If an uninsured driver damages your vehicle. If an uninsured motorist damages Moose, I will just ask him to pay for everything, otherwise, I will introduce him to my cousin Viktor, who will break his legs for non-payment.

Towing & Labor:?Provides emergency roadside assistance such as towing, flat tire, jump start, and delivery of gas. This service is a life saver! I?ve run out of gas, had an older car not start before for some reason, blew out my tire, left my lights on, and each time needed roadside assistance. Takes no more than 45 minutes to get everything fixed. I recommend everybody get towing & labor. I pay only $5.88 and I can?t believe it?s not at least $100 every 6 months.

AUTO INSURANCE RECOMMENDATIONS BY FINANCIAL SITUATION

Given everybody should have auto insurance, it is now up to you to taylor your auto insurance coverage to your needs. Here are four common scenarios and what type of insurance I recommend for each.

1997 BMW M3

* You drive a fancy car which you probably can?t afford. Let?s say you are 28 years old, no family, and drive a new 2013 BMW 335i that costs $57,000. The problem is you only make $60,000 a year and therefore can?t really afford the car if you follow my 1/10th rule of car buying (pay no more than 1/10th your gross annual income for a car). Since you can?t afford the car and only really the payments, you probably can?t afford any major repairs. My recommendation is to bite the bullet and get comprehensive auto insurance, collision, towing & labor, and uninsured motorist and property damage insurance. Since your car is literally making you poorer every month, you don?t need to get as much property and bodily injury liability insurance ironically. Your car is already a massive financial liability. You don?t want to make it an even greater liability without comprehensive coverage.

Estimated cost: $200-$300 a month depending on driving record and liability amounts.

* You drive a nice car and are rich. Let?s say you drive a 2013 Honda Accord that costs $38,000. You also make about $400,000 a year, have $3 million in assets, a wife, two daughters in high school, and paid cash for your Accord. You care most about insuring your passengers and your financial assets in case you are deemed negligent. The last thing you want is someone to sue you for $3 million dollars and put your family at risk. College is expensive! Hence, I would consider as much bodily injury and property liability insurance as possible. You should probably get comprehensive insurance with a higher deductible of around $1,000 given you have the assets. With a higher deductible, your monthly premiums are less. It is worth it to also get roadside assistance, collision, property damage, and uninsured motorist insurance. You should also consider an umbrella policy to cover you beyond your auto insurance.

Estimated cost: $50-$150 a month depending on driving record and liability amounts.

* You drive a beater, but can afford more. This is my situation where I drive a $3,500 dollar 12 year old SUV, but can afford something newer. I don?t own a nicer vehicle because I don?t sell my children and I?m pretty frugal, especially now that I?m retired. I do not have comprehensive insurance because if I bash Moose by mistake, I?ll just buy another car. If someone else bashes Moose, I?ll be mad, but at least his/her insurance will pay to replace Moose. My main concern is about getting sued if I?m at fault given I?ve got a good accumulation of assets in CDs, savings, private investments, stocks, bonds, and real estate. Hence,?I elected for $300,000/$500,000 per person/per accident for bodily insurance liability. I?m assuming $300,000 per person is enough and hope it never comes to it. I have a multi-million dollar umbrella policy.

Estimated cost: I pay about $45 a month.

* You drive a beater and are poor. Let?s say you have a non working spouse, no children, and drive a 1993 Honda Civic worth $1,000, make $28,000 a year and have a total of $500 in net worth. I recommend you just get liability insurance with the lowest monthly premiums, and lowest liability payouts possible. The most anybody will be able to get from you is $500, which is not worth the hassle for most people. Spending $10 more a month in auto insurance is painful, hence your goal is to keep costs low, be a safe driver, and don?t get into any accidents. Remember, if someone else is found liable in your car accident, it is up to them to pay you, not the other way around.

The right amount of insurance is different for everybody. Make sure you understand the various different options you have and weight the cost of coverage with the cost of your monthly premiums.
